YEARLY!             All athletes/parents must complete the athletic clearance process by registering on www.homecampus.com/clearances. Once registered, you may login and complete all the necessary steps for clearance. In addition to these clearance steps, Pacifica will also need to have a copy of the physical examination on file. You may either drop off a hard copy to the front office OR scan a copy into your profile on athleticclearance.com. A physical examination form is available for download below OR on athleticclearance.com.
